Welcome To the most common type of diabetes, is a disease that occurs when your
blood glucose, also called blood sugar, is too high. Blood glucose is
your main source of energy and comes mainly from the food you eat.
Life is not over because you have diabetes your body doesn't make enough insulin or can't use it as well
as it should. When there isn't enough insulin or cells stop responding
to insulin, too much blood sugar stays in your bloodstream.

Our Process

Upon completing the referral form on our website, we will reach out to you to ask questions and to set up an intake and / or information meeting. We will be there to support your program goals whether it be securing a job and maintaining employment or developing the skills to live independently. Progress is measured and results are discussed at team meetings. Your goals may be re-evaluated, and new goals established to support desired outcomes.

WHAT IS HSS?

Housing Stabilization Services (HSS) is a new Home and Community based Service (HCBS) that supports people with disabilities in finding housing; setting a basic living budget; creating natural community supports by connecting with landlords and neighbors; and assisting people with understanding the rules and responsibilities of a lease.
